Course Overview

This three-day course focuses on the team approach to addressing issues. Participants will understand the methodologies used in root cause analysis and the importance of using the correct tools during a root cause investigation. By discovering how to utilize these tools, as well as the scientific method and statistics, participants will be able to implement root cause solutions into their organizations. At the completion of this course, participants will be able to:

  • Define Root Cause Analysis (RAC)
  • Analyze different approaches to Root Cause Analysis
  • Evaluate existing problem-solving models
  • Understand how to diagnosis problems and incorporate the correct responses
  • Understand when and how to apply filters for diagnosis
  • Identify the five steps for diagnosing problems including: Defining the problem, understanding the process, identifying possible causes, collecting data and analyzing data.
  • Display the root cause problem-solving processes using a Flowchart
  • Determine how to organize and display causes using other charts including: Logic Trees, Cause and Effect Diagrams, Barrier Analysis and change analysis
  • Recognize Cause and Effect when diagnosing problems
  • Determine best practices for data collection
  • Identify different tools for both high frequency and low frequency problems
  • Identify which tools can be used for the analysis process
  • Evaluate how solutions will affect processes and implement the best solutions
  • Interpret data and communicate information by creating data analysis summaries
  • Understand and review causes for Human Error
  • Address issues that may arise when working with the collaboration team including: cognitive bias, emotional barriers, organizational changes, project responsibilities and coaching/facilitation issues
  • Determine how critical thinking is used in improving corrective actions
  • Prevent future problems by establishing corrective action and areas of improvement
